GP Management Plans
A GP Management Plan enables a GP to plan and coordinate the health care of patients with chronic or terminal medical condition. They include patients with these conditions who require multidisciplinary, team-based care from a GP and at least two other health or care providers. A chronic medical condition is one that has been (or is likely to be) present for six months or longer, for example, asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke. There is no list of eligible conditions; however, the Chronic Disease Management items are designed for patients who require a structured approach, including those requiring ongoing care from a multidisciplinary team.
Eligibility for a GP Management Plan is accessed by a GP taking into account a patient’s medical condition and care needs, as well as a strict set of guidelines set out by the Department of Health. A GP Management Plan gives patients access to free or subsidised allied health services. All of the doctors at our surgery are able to access a patients eligibility for a GP Management Plan. It’s also a great way for patients to be able to get Medicare rebates for the additional services that they need.
